PG medicine abroad for Indian doctors is mostly a licensing and residency process, not a college admission. In the USA you pass USMLE and ECFMG certification and enter residency through the Match. In the UK you register with the GMC and build a career through NHS jobs and specialty training. In Germany you earn your Approbation and join a paid Facharzt residency. None of these routes uses NEET PG.
In India, a PG seat means an MD or MS through NEET PG. Abroad, the system is different. Countries license you to practise, and you then train as a salaried resident doctor inside a hospital until you become a specialist. You earn during that training, and you finish with a specialist qualification recognised in that country.
This guide compares the three routes Indian doctors ask about most: the USA, the UK and Germany. Other countries such as Ireland, Australia and Canada have their own registration exams, and we can walk you through those on request. Whatever you choose, plan for it early. Language exams, licensing exams and document verification each take months.
| Factor | USA | UK | Germany |
|---|---|---|---|
| Entry step | USMLE Step 1 and Step 2 CK, plus ECFMG certification | GMC registration through PLAB or an accepted qualification | Approbation through language exams and the Kenntnisprüfung |
| Language | OET Medicine for the ECFMG pathway | IELTS 7.5 overall with 7.0 in each part, or OET grade B | General German B2, medical German close to C1 |
| How you get a training post | ERAS application and the Match | NHS jobs first, then specialty training applications | Direct applications to hospitals |
| Training length | 3 to 7 years | Depends on the specialty | Around 4 to 6 years or more |
| Biggest hurdle | Competition in the Match | UK graduate priority for training posts from 2027 | Language and the knowledge exam |
The US route runs through ECFMG, the body that certifies international medical graduates. You must pass USMLE Step 1 and Step 2 CK, verify your medical degree, complete an ECFMG pathway that covers clinical and communication skills, and take OET Medicine as your English test. You then apply for residency through ERAS, and programmes fill their places through the National Resident Matching Program.
ECFMG's Recognized Accreditation Policy also looks at whether your medical school is accredited by an agency recognised by the World Federation for Medical Education, and it mainly affects recent and future graduates. Check your school's status early, and check the current pathway dates on the ECFMG website, since the application windows are short.
Be realistic about the odds. Match rates for non-US international graduates are commonly quoted at roughly 55 to 60 percent, and higher for specialties such as internal medicine than for competitive ones like dermatology or orthopaedics. Preparation, exam fees, US clinical experience and application costs often add up to a five-figure dollar sum, so budget before you commit. Most residents train on a J-1 or H-1B visa arranged through the programme.
The UK route starts with GMC registration. You show your medical qualification, pass the GMC's clinical tests or hold an accepted postgraduate qualification, and prove your English with IELTS Academic at 7.5 overall with at least 7.0 in each part, or OET at grade B. AbroadUp's IELTS preparation course helps you plan for that band. GMC registration then lets you work in the NHS.
The bigger story in 2026 is the Medical Training (Prioritisation) Act. From the 2027 offers onward, specialty training places must first go to applicants prioritised in regulations, which include UK graduates and doctors with significant NHS experience. Applications for specialty training rose from about 12,000 in 2019 to nearly 40,000 in 2026 while posts stayed broadly flat, which explains the policy.
The Act does not change GMC registration or the exams, and it does not cover non-training NHS posts such as trust grade, clinical fellow and specialty doctor roles, which still employ many international doctors. It does mean that direct entry into a UK training programme is harder than it used to be, so many Indian doctors now plan for a few years of NHS service posts, exams such as MRCP or MRCS, and then applications. Read our page on studying and working in the UK for the wider picture.
Germany does not run classroom MD or MS programmes. You become a specialist (Facharzt) by working as a salaried resident doctor in a hospital under the state medical chamber. There is no NEET PG style exam, and you apply directly to hospitals. Training usually lasts four to six years or more, and you are paid throughout.
The path has a clear order. Your Indian degree is assessed against the German curriculum, and most Indian graduates receive a notice that they must take the Kenntnisprüfung, an oral and practical knowledge exam covering internal medicine, surgery and one further subject. You need general German at B2 to start the application and medical German close to C1 to pass the Fachsprachprüfung, which tests patient conversation, documentation and doctor-to-doctor talk. Many doctors work under a temporary permit (Berufserlaubnis) while they prepare for the Kenntnisprüfung, then receive full Approbation.
Commonly quoted starting salaries for a resident doctor run from about €4,500 to €6,000 gross a month depending on the hospital, state and collective agreement, and many guides estimate 12 to 24 months from your first German class to Approbation. Treat both as ranges. AbroadUp's German language courses and German language classes in Delhi cover A1 to C2, and our note on how long it takes to learn German in India will help you plan. For the wider route, see our Germany destination page.
The USMLE route and the German route reward different strengths. The first rewards exam performance, and the second rewards language commitment. Be honest about which one you can sustain for a year or more.

Book Free Counselling →Visas depend on the country and the job offer. US residents usually train on a J-1 or H-1B arranged by the programme. NHS doctors normally come on a work visa linked to a sponsoring employer. In Germany, many Indian doctors first enter on a recognition visa or the Chancenkarte while they prepare for exams, and then move to a work permit once a hospital hires them. Rules and salary thresholds change, so confirm them at the time of application. AbroadUp's visa and services team checks your documents before you file.
If you are also considering the US as a wider destination, see our USA education consultant page for how we support applicants there.
No. The US, UK and German routes do not use NEET PG. You qualify through licensing exams, language tests and hospital applications, and you train as a salaried resident doctor.
Yes. Germany has no classroom MD or MS, but Indian doctors can become specialists through a paid Facharzt residency after Approbation. You need general German at B2, medical German close to C1, and usually the Kenntnisprüfung.
The GMC asks for IELTS Academic at 7.5 overall with at least 7.0 in each part, or OET at grade B in each part. Check the GMC website for the current requirement.
No. GMC registration and the exams are unchanged, and non-training NHS posts are not covered by the Act. What changes is that UK graduates and doctors with significant NHS experience are prioritised for specialty training offers from 2027.
It is competitive. Match rates for non-US international graduates are commonly quoted at roughly 55 to 60 percent, and they vary a lot by specialty. Strong Step scores, US clinical experience and early planning improve your chances.
Commonly quoted starting salaries range from about €4,500 to €6,000 gross a month, depending on the hospital, state and collective agreement. Night and weekend duties add to that.
It depends on your strengths. The USA rewards exam scores and carries Match risk, the UK offers NHS work but has tighter training access, and Germany rewards language commitment with a paid residency and no Match. A profile review helps you choose.
